Coordinated Care first meeting May 7

Bridges Out of Poverty of Sublette County is sponsoring a new group called Coordinated Care working towards building a strong, healthy, connected community. Interested people are invited to join local nonprofits, human services, and individuals to share what we are doing, see where we can collaborate, help and promote one another, learn, and generally network. Meetings will be held the first Wednesday of every month in Room 120 at Sublette BOCES in Pinedale at noon. Lunch will be provided. The first meeting will be Wednesday, May 7th.
