Description:Summary Description:
While upholding the Sublette County Hospital Districts mission, vision and values, the Skilled Nursing Facility the Registered Nurse coordinated and delivers, on an age and developmentally appropriate basis, direct clinical care per nursing process for adult and geriatric residents/patients. The RN shall utilize knowledge of residents’ rights, legal/ethical concepts, communication skills, safety and infection prevention while performing the following: basic nursing skills, basic restorative skills, mental health and psychosocial skills, and nursing team member skills.

Supervision Received: Directly supervised by the Director of Nursing.

Responsibilities:

Administrative:

Provides direct supervision and coordination of the resident care team. Is responsible for the quality of care given by self and the resident care team.

Participates in patient and resident care through procedures within the scope of their training and practice, according to state and federal regulations and facility policies and procedures.

Assures that quality of care is given to each resident by supervising, directing, and motivating the CNA staff in their charge according to the plan of care and policies and procedures.

Reviews CNA’s resident assignments and duties at the beginning of the shift and performs ongoing evaluation of residents needs and communicates to the CNAs throughout the shift.

Provides feedback to the DON regarding performance of nursing assistants or other care staff.

Communicates staffing problems and needs to the Director of Nursing.

Supports and maintains a culture of safety and quality.

Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.

Meets with members of the interdisciplinary team for development, implementation, and evaluation of residents’ care plans and protects resident from neglect and abuse.

Promotes residents rights, provides interventions to increase residents’ quality of life and self-worth and involves resident in decisions affecting their care.

Consistently involves resident and family in planning for care, treatment, teaching needs, and discharge plans. Initiates and leads resident care conferences and updates plan of care, as appropriate.

Functions as a leader for the CNAs and LPNs/LVNs by delegating in a manner that contributes to excellence if the nursing unit.

Clinical:
Assesses residents on admission ensuring that the nursing assessment is completed within 24 hours and all other assessments are completed within 72 hours. Submitting all documentation in a timely, clear, concise, and pertinent manner.

Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of residents and their families.

Provides direct patient care, according to physician’s orders and individual resident’s needs, including administration of medications, treatments, therapies and performs and assist with ADL’s, in accordance with recognized standards of practice and organizational standards.

Identifies and reports changes in condition to the attending or on call physician, the DON, and the responsible family member. To include documentation of this change in the progress notes for the resident’s medical record.

Ability to assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.

Assesses residents and completes monthly nursing summary as well as the quarterly assessment and documents appropriately including Minimum Data Sets.

Notes, carries out or ensures that physician’s orders are performed in a timely manner.

Demonstrates ability to interpret results of waived tests; takes appropriate action on waived test results or any abnormal lab or radiology results.

Follows proper infection prevention and control procedures and supervises supportive staff in infection prevention and control techniques.

Understands the policies on restraints their proper use.

Actively receives change of shift report and knows location of assigned residents.

Gives a thorough report to the oncoming shift to ensure the unit is left in good condition and oncoming shift knows assigned residents and their status.

Follows through, as needed, on information given during shift report, by resident, or by family regarding resident concerns.

Provides all the above general activities to residents in isolation.

Adheres to all policy and procedures of Sublette County Health and the Nursing Department.

Attends staff development activities to improve knowledge and skills and to maintain competency and certification.

Maintains personal health and wellness to prevent transmission of illness to patients or residents.

Other duties as assigned by Director of Nursing of SCH

The job holder must demonstrate current competencies applicable to job position.

Qualifications:
Graduate of an accredited school of Nursing.

Current unrestricted license as a Registered Nurse in the State of Wyoming.

Minimum of one-year experience in geriatric care preferred.

Current certification in Basic Life Support or certification within 3 months of hire.

Knowledge, Skills, and Abilities:
Possesses a genuine interest and concern for adults in later years of life.

Demonstrates excellent verbal and written communication skills.

Knowledge of long-term care procedures and goals.

Ability to work as a member of the "health care team."

Knowledge of good body mechanics and proper lifting techniques.

Knowledge of performing and following infection prevention and control procedures.

Ability to follow instructions and ask questions as appropriate.

Demonstrates the ability to remain calm in emergency and/or stressful situations.

Ability to communicate clearly and effectively with residents, visitors, and coworkers.

Working Conditions:
Subject to varying situations including ability to understand and respond appropriately, effectively, and sensitively to special population groups, including those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, etc.

Frequent contact with a variety of people including prisoners and people with mental, emotional, and physical disabilities.

Medical office, exam room, laboratory, radiology, emergency room and professional practice settings.
Frequent exposure to communicable diseases and other conditions common to rural clinic environments.

May require travel between District sites.

Physical Demands:
Varied activities including moving, reaching, bending, and lifting.
Requires full range of body motion including handling, moving, positioning, and transporting patients. Also requires manual and finger dexterity, and eye-hand coordination. Requires standing and moving for extensive periods of time. Occasionally moves items weighing up to 50 pounds without assistance. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.
